For your convenience, we have provided our patient forms on this website for you to download and print out. In preparation for your first appointment, please download and print the Consent for Physical Therapy Treatment Form, the HIPAA Notice of Privacy Acknowledgement Receipt Form, the Financial Policy/Cancelation Policy Form, and the Workers' Compensation/Auto Injury Liability Form. Please complete all forms and bring them to your appointment.
Please also bring your photo ID and health insurance card(s) to your appointment.
If you are being treated for a work or auto-related injury, please also download, print, and complete the Work Comp/Auto Liability Form. You will also need to bring your auto insurance card, claim information, and date of accident. Patients being seen for work or auto-related injuries should be printing all of the forms seen below.
